I, the undersigned parents or guardian, hereby consent to my child(ren), listed below, to participate in the activities connected with the trip to and from and the time at the Carnival at Central Baptist Church, Baton Rouge, LA on April 6, 2024. I certify that my child is able to participate in these activities. If my child has medical conditions which may be relevant to a physician in the event of an emergency, I have listed below. In the event an emergency occurs, I may be reached at the telephone number listed below. If I cannot be reached, I hereby authorize an adult sponsor(secondary emergency contact) listed below to make emergency medical decisions for my child. If there are any activities I do not want my child to be involved in, I have listed them below.

I UNDERSTAND AND HEREBY AGREE TO ASSUME ALL OF THE RISKS WHICH MAY BE ENCOUNTERED ON SAID ACTIVITY, INCLUDING ACTIVITIES PRELIMINARY AND SUBSEQUENT THERETO. I do hereby agree to hold Roundup Ministries, Kevin Walker and Central Baptist Church and its agents and employees, harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property, even injury resulting in death, which I now have or which may arise in the future in connection with the activity or participation in any other associated activities.

I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of Louisiana and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto and the terms of this release are contractual and not a mere recital.

I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF AND I SIGN THIS RELEASE AS MY OWN FREE ACT. This is a legally binding agreement which I have read and understand.



I recognize the risk of injury inherent in or incidental to being in the proximity of horses, or the handling and riding of horses due to their size and unpredictability and I voluntarily assume this risk and release Central Baptist Church pastor, deacons, trustees, workers and member from any or all liability for injuries, including serious injury or death. I shall bring no claims, demands, actions, or causes and/or litigation against the above named parties or individuals or business for any economic or non-econimic loss/losses due to physical harm, mental harm or anguish, bodily injury, death or property damage sustained by me, my property, or my minor children/legal wards listed above, caused by animal, horse, tack, or equipment at or in relation to the premises and operations of Central Baptist Church (including but not limited to the trails, woods, drainage ditches, and associated areas of debris, and any and all manner of structures including but not limited to fences, utility poles, buildings, or other potentially dangerous materials) while walking, riding, or handling horses and/or otherwise being in the vicinity of any/all animals, poisonous, plants, on the premises.




Under Louisiana law, a farm animal activity sponsor or farm animal professional is not liable for an injury to, or the death of a participant in farm animal activity resulting from the inherent risks of the farm animal activity, pursuant to RS.9:2795.1.
